Medicare GPCCMP allied-health sessions

From 1 July 2025 the GP Chronic Conditions Management Plan (GPCCMP) replaces the older GP Management Plan / Team Care Arrangement. Here's how patients access up to five subsidised allied-health visits.

  • Your GP prepares a GPCCMP and refers you to eligible allied-health providers.
  • Up to 5 subsidised visits per calendar year across all allied-health providers combined.
  • Medicare pays a rebate; there is usually a small gap or bulk-billing depending on the clinic.

What changed

From 1 July 2025 the GPCCMP replaces the GP Management Plan (item 721) and Team Care Arrangement (item 723). Patients with an existing plan continue on their current arrangement until it's reviewed, then transition.

Getting the referral

Book a chronic-disease management appointment with your GP. If a plan is prepared, you'll receive referral forms for the allied-health providers you'll see (e.g. physiotherapy, exercise physiology).
