Home Visit Physiotherapy and Exercise Physiology
When attending the clinic is difficult, our physiotherapists and exercise physiologists may be able to provide assessment and rehabilitation in your home.
Home visits are available in selected Blacktown and Western Sydney suburbs, subject to clinician availability, travel arrangements, clinical suitability and funding approval.
Who may benefit?
Home visits may be considered for people who:
- Have disability-related mobility limitations
- Receive NDIS supports
- Receive Support at Home services
- Are participating in the Restorative Care Pathway
- Are recovering after surgery or hospitalisation
- Have an approved workers compensation or CTP claim
- Have a high falls risk
- Find transport or clinic access difficult
Services at home
Depending on the discipline and assessment, a visit may include:
- Mobility and functional assessment
- Strength and balance exercises
- Falls-prevention strategies
- Walking and transfer practice
- Post-operative rehabilitation
- Individual home exercise programs
- Exercise for disability or chronic conditions
- Education for participants, families and carers
- Review of progress and adjustment of the program
Some assessments or exercises may be better completed in our rehabilitation gym. Your clinician will discuss whether home, clinic or combined appointments best suit your needs.
Arranging a home visit
Please provide:
- Participant or patient name and contact details
- Home address and suburb
- Requested discipline
- Funding type
- Reason a home visit is required
- Mobility, access or safety information
- Relevant referral or approval documents
Travel charges, service areas and cancellation arrangements will be explained before the first visit.
